Attention All Non-Medicaid Providers for BHSD:

Important changes to the Critical Incident Reporting (CIR) process, effective July 1, 2017

A critical incident is a reportable incident that may include, but is not limited to, abuse, neglect, or exploitation, death; environmental hazards, or law enforcement intervention that encompasses the full range of covered services. Prior to July 1, 2017, all non-Medicaid CIRs were submitted to Optum Health New Mexico (OHNM). Starting July 1, 2017, OHNM will no longer be the Administrative Services Organization (ASO) for the State of New Mexico Interagency Behavioral Health Purchasing Collaborative.

Critical incident reporting for non-Medicaid constituents

As of July 1, 2017, all CIRs for non-Medicaid constituents should be faxed to State of New Mexico Interagency Behavioral Health Purchasing Collaborative using a new CIR form and fax number.

Continue to follow the instructions found in the Behavioral Health Critical Incident Reporting Protocol (September 2015). Use the revised form (CIR form Appendix A, July 2017) that includes the Collaborative fax number 505- 476-9272.
